Description
This pyramid shows the cascading effect of bad decision making and how it can ultimately result to fatalities within the workplace of CSX. A foundation of 30,000 unsafe conditions supports circumstances that can lead to 3,000 accidents resulting in minor or no injury. However, 10% of those accidents could result in lost time, restricted duty or medical treatment, and further subsets could lead to disabling, or even death.
The spurious numbers certainly present a low mortality rate with little factual evidence to back it up, but that was probably the point, wasn’t it?
